Optimizing post-discharge support for secondary prevention in coronary heart disease: what can implementation science bring to the table?

Post-discharge care for patients with coronary heart disease is often fragmented meaning that not all patients receive evidence-based care for secondary prevention which negatively impacts patients, healthcare providers, and health systems. In this interactive 2-hour session, we will highlight some of the key challenges to implementing evidence-based practices post-discharge and will cover topics such as transitional care, treatment adherence and cardiac rehabilitation, and virtual care innovation. The overall aim for this session is to connect individuals interested in this topic area and begin building an implementation-focused multidisciplinary research network to improve care for patients with coronary heart disease.

This session will be hosted by the Centre for Evidence-Based Implementation (CEBI; McMaster University, Hamilton, Ontario, Canada), and will take place on Thursday, December 16, 2021, from 8:00-10:00 a.m. EST. Although this session is an adjunct event to the virtual 14th Annual Conference on the Science of Dissemination & Implementation in Health, you do not need to be registered for the conference to participate in this session.
